• LU 420 BTJ Steamfitter Apprentice Evaluation Form

    Help the LU 420 training school to​ improve their program by submitting an apprentice evaluation form. It is requested that you submit a form once a period and/or at the point of a layoff or termination.
    Attendance
    Can be depended on to attend work regularly.
    Punctuality
    In reporting for work and returning from lunch, etc.
    Cooperation
    Willingness to follow orders from and work with other mechanics and foremen.
    Initiative
    Works on the next task without having to be told.
    Mechanical Ability
    Rating of mechanical ability considering his/her experience.
    Performance
    Willingness to work hard all day.
    Probability of Success
    How would you rate his/her chances of becoming a top level mechanic.
    Attitude
    Rate his/her seriousness to learning the trade and responsibility to the employer.
